Rural Health Network

HCA is one of 32 Rural Health Networks across New York State funded by the New York State Department of Health Charles D, Cook Office of Rural Health. As a Rural Health Network our mission is to  improve the quality of life in rural communities through broad-based inclusive partnerships that support wellness and prevention.  Our network is a collaboration among diverse organizations and constituencies who work together on action-oriented opportunities at a local, regional and state level.  Network members represent a variety of participants, including (but not limited to) healthcare providers, nonprofit organizations, government agencies, public health professionals, educational providers, and private organizations.
