Our Rural Health Network
Healthy Community Alliance’s mission is to improve the quality of life of rural communities through broad-based inclusive partnerships that focus on wellness and prevention.
Since it first received funding from the New York State Charles D. Cook Office for Rural Health in 1994, Healthy Community Alliance (HCA) has worked tirelessly to address systemic barriers to health access in rural areas, including geographic isolation, limited resources, and socioeconomic disparities. By fostering partnerships and implementing innovative programs tailored to the needs of the community, HCA has been successful in launching sustainable solutions to ensure equitable access to services, particularly for underserved populations.
HCA was incorporated in 1996 as a 501(c)3 non-for-profit corporation and is one of 31 Rural Health Networks (RHN) across New York State. New York State’s Rural Health Network Development Program is the foundation of our funding to help address rural health issues across our service area.
Today, Healthy Community Alliance has a growing network committed to local partnership and innovation for the advancement of rural health equity. We work together with our partners to identify gaps in services, potential funding sources, areas of collaboration, and forums for shared learnings.
HCA directly impacts the community through its ownership of Community Place and Academy Place Apartments, and in its operation of programs such as health insurance counseling and navigation, senior and caregiver support, maternal/infant home visiting, WIC and SNAP benefits enrollment, and health coaching. Healthy Community Alliance continues to be recognized as a preeminent rural health network committed to health promotion, disease prevention and building quality of life.

Our service area encompasses Cattaraugus County and parts of Chautauqua, Erie and Wyoming counties. Specific program service areas may depend on funding criteria and can be found on program pages.
