The Personal Wellness Profile is a computerized health assessment tool consisting of a multiple choice questionnaire which is electronically scanned and processed. Personal Wellness Profiles

The Healthy Community Alliance has used this survey in local school districts to help determine curriculum designed to improve health and wellness among students. 

Each participant receives a confidential Personal Wellness Profile which rates their wellness on a scale from "poor" to "excellent".

Group reports aid  in designing programs based on need. PWP surveys conducted between 2003 and 2005 reveal significant issues in the areas of nutrition awareness and physical activity.

The Alliance also makes PWP services available to organizations and employers   interested in assessing the wellness of its members or employees. These services are provided at a reasonable fee and can be arranged by contacting the Healthy Community Alliance.
